This paper deals with one of the key problems of e-healthcare which is the security. Patients are worried about the confidentiality of their electronic medical record (EMR) which could be used to expose their identities. It is high time to revisit the confidentiality and security issues of the existing telehealth system. Intruders can perform sniffing, spoofing, or phishing operations effortlessly during the online exchange of the EMR using a digital platform. The EMR must be transmitted anonymously with a high degree of hardness of encryption by protecting the authentication, confidentiality, and integrity criteria of the patient. These requirements recommend the security of the current system to be improved. In this paper, a neural synchronization-guided concatenation of header and secret shares with the ability to transmit the EMR with an end-to-end security protocol has been proposed. This proposed methodology breaks down the EMR into the n number of secret shares and transmits to the n number of recipients. The original EMR can be reconstructed after the amalgamation of a minimum k (threshold) number of secret shares. The novelty of the technique is that one share should come from a specific recipient to whom a special privilege is given to recreate the EMR among such a predefined number of shares. In the absence of this privileged share, the original EMR cannot be reconstructed. This proposed technique has passed various parametric tests. The results are compared with existing benchmark techniques. The results of the proposed technique have shown robust and effective potential.Pure large cell neuroendocrine carcinoma of the gallbladder is a rare disease. However, the prognosis of this aggressive tumor is poor with short survival after diagnosis. We are describing in this manuscript a case of pure large cell neuroendocrine carcinoma in which survival exceeded 26 months, after performing two curative surgeries. We are reporting the case of a 68-year-old woman with a history of recovered right breast carcinoma and operated 6 years later for a completely asymptomatic gallbladder tumor of 31 mm. In this case, curative surgery was performed allowing monobloc resection of the gallbladder and the hepatic segments IVb and V, a lymph node dissection was performed to. The histological examination of the specimen and immunohistochemistry confirms that the tumor was a grade 3 pure large cell neuroendocrine carcinoma of gallbladder with lymph node invasion, the hepatic and biliary surgical margins were free. Postoperative adjuvant chemotherapy was administered and the evolution was eventless until the discovery at 20 months of a lymph node considered being metastatic recurrence. A second surgery was performed allowing removal of three lymph nodes. This time, a different protocol of chemotherapy was administered to our patient who remains alive and without recurrence at 26 months from her first surgery. Surgical relentlessness with free margins resections associated with appropriate chemotherapy probably improves the survival of patients suffering from this rare and aggressive tumor.There is an unprecedented array of new satellite technologies with capabilities for advancing our understanding of ecological processes and the changing composition of the Earth's biosphere at scales from local plots to the whole planet. We identified 48 instruments and 13 platforms with multiple instruments that are of broad interest to the environmental sciences that either collected data in the 2000s, were recently launched, or are planned for launch in this decade. We have restricted our review to instruments that primarily observe terrestrial landscapes or coastal margins and are available under free and open data policies. We focused on imagers that passively measure wavelengths in the reflected solar and emitted thermal spectrum. The suite of instruments we describe measure land surface characteristics, including land cover, but provide a more detailed monitoring of ecosystems, plant communities, and even some species then possible from historic sensors. The newer instruments have potential to greatly imates of water and carbon budgets. We also include a few spaceborne active LiDAR and radar imagers designed for quantifying surface topography, changes in surface structure, and 3-dimensional canopy properties such as height, area, vertical profiles, and gap structure. We provide a description of each instrument and tables to summarize their characteristics. This synthesis method comprises the preparation of GQDs via hydrothermal route, which should be conjugated to the LCysteine functionalized core-shell magnetic structure with the core of about 7.5-nm iron oxide nanoparticle and 3.5-nm LCysteine shell. LCysteine, as a biocompatible natural amino acid, was used to link magnetite nanoparticles (MNPs) with GQDs. X-ray powder diffraction, Fourier-transform infrared spectroscopy, X-ray photoelectron spectroscopy, energy dispersive X-ray were used to investigate the presence and formation of MNPs, L Cysteine functionalized MNPs, and final hybrid nanostructure. Morphology and size distribution of nanoparticles were demonstrated by scanning electron microscopy and transmission electron microscopy. Finally, the magnetic and optical properties of the prepared nanocomposite were measured by vibrating sample magnetometer, ultraviolet-visible, and photoluminescence spectroscopy.
